In today’s impersonal healthcare environment, where patients are consumers and nurses are regarded as just another “service provider,” I strive to humanize the practice of nursing and bring back the warmth and personal attention by providing personalized and customized home nurse care to my geriatric patients. I make it possible for my elderly patients to stay at home for as long as possible, empowered by the best quality of life regardless of any medical condition they are facing.
How do I do this?
In my home nurse practice I do not follow the mainstream paradigm of nursing which is mainly to treat the RESPONSE that the patient has to an illness. I add onto this the philosophy of nursing that focuses on the WHOLE PERSON, by also being sensitive to the needs and feelings of the patient and her family. And the only way to provide this type of care is to give the gift of quality time and personal attention AT HOME, working with only one patient at a time.
My practice is to dedicate myself to a small number of patients, which allows me to develop trust with them and their families. I accomplish this by thoroughly listening, discussing and recognizing each family’s needs and stated desires to personalize their care. No two families are alike and neither should be their health treatment. My preference is to take on long-term cases, greater than six months, to provide continuity of care to my patient that can have lasting effect on her quality of life.

These home nurse services come bundled in a three-for-one package. Incorporating these three services is what I’ve come to learn brings the best and most satisfying results for my patients and their families, allowing them to enjoy their journey with each other for as long as is naturally possible.
Description of Home Nurse Services:
- Customized Licensed Home Nurse Services: Includes the planning, organizing and provisioning of home nurse care tailored to meet the specific needs of the patient. This I accomplish through thorough interaction with each patient and her relatives to document all critical needs and personalize care in terms of goals to achieve during the span of service.
- Case Management Services: Here I coordinate overall interdisciplinary planning for the care for my patient, serving as liaison between patient and family, healthcare providers and home-care personnel, to ensure a continuity of quality effectively and promptly served.
- Dedicated Concierge Home Nurse Services: Under this unit, my function is to provision a patient-centered care that focuses on the whole person, assisting my patient and her family to navigate through the time-consuming healthcare system to ensure access to full care commitments from the various health service providers.
As an RN I am covered with a professional liability insurance, and maintain on-going accreditation through continuous education.
Regrettably I do not serve clients wholly dependent on managed care or insurance plans.

Personalized Plan of Care
- Pre and post operative patient education and care
- Stay with patient in hospital
- Catheter, IV, PICC and Mediport line maintenance
- Antibiotic and IV IG infusions
- Surgical site wound care
- Wound care and dressing changes
- Potential infection assessment
- Safely dispense and manage all medications including refills, renewals
- Maintain up-to-date medication list with schedule and side effects
- Monitor fluid input/output and vital signs
- Regular injections
- Home safety assessment and recommendations for equipment and/or renovations
- Oversight and coordination of physicians
- Facilitate communication between specialty physicians
- Escort to and from doctor visits and procedures
- Order and/or arrange for outside services such as delivery of meds, medical equipment and supplies
- Acquisition and supervision of outside contractors for health and home care
- 24/7 availability for any emergency
- Temporary live-in, when necessary
- Travel companion
- Chaperone to local events or places of interest for the client
- Frequent or on-demand reporting to relatives on the status of the patient
- Liaison between patient, distant relatives and medical staff
